Staff 3 (S3) was in Room 7 to assist S1 at approximately 11:50AM-12:15PM. When S3 left, they recalled seeing S1 assisting C1 to lay down for nap time. There were no other staff members present when S3 left Room 7. S3 stated that C1 bumped their head on the cabinet while they were on break.
During record review, LPA obtained the incident report, staff time cards, and the daily attendance sheet from 05/06/2026. The incident report confirms that C1 had bumped their head on the cabinet, and their authorized representative was notified at 12:44PM. The attendance sheet confirms that there were fourteen (14) children present in Room 7. The employee time cards show that S1 was working from 8:25AM-1:08PM while Staff 4 (S4), S1's co-teacher in Room 7, was on break from 11:52AM-12:56PM.
Based on staff interviews and record reviews, the facility was not in compliance with California Code of Regulations, Title 22, Division 12 & Chapter 1, 101230(c) Activities is being cited; see LIC 809-D. It was determined that one (1) Type A citation was cited during today’s visit.
LPA Kathy Trinh informed Director Michelle Gjertsen that this report dated 05/15/2026 for Type A citation shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Kathy Trinh informed Director Michelle Gjertsen to provide a copy of this licensing report dated 05/15/2026, Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, 05/15/2026.
Appeal Rights discussed. Director was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
