An informal office meeting was conducted on this date, 6/10/2025 in the Orange County Regional Office. In attendance was Licensing Program Manager (LPM), Thuy Ho, Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek, and Oak View Preschool representatives , Luisa Tanzil (Early Learning Supervisor), and Elaine Burney, Coordinator of Early Learning Program. The purpose of the informal meeting was to discuss the following violation which was issued on 5/28/2025.

Responsibility for Providing Care and Supervision- No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The above requirement was not met as evidenced by staff not noticing a 3-year-old child exiting the classroom to the parking lot, being left unattended for approximately 60 feet away from the classroom for a couple of minutes until found by A1, and A2 in the parking lot. This is an immediate risk to the health and safety of children in care.

On 5/29/2025, the facility representative, Luisa Tanzil provided documents and information required pursuant to the plans of correction. They stated that the acknowledgment of receipt of the licensing reports, signed by each parent in both morning and afternoon sessions at Oak View Preschool, and were filed in each child’s individual file.
They confirmed that the safety measures outlined in the plan have been in place. Additionally, door alarms have been purchased online and will be installed as soon as they arrive. They included the following in their plan of correction.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

1- Drop off and pick up spot for morning and afternoon sessions. Parents enter through the gate located beside the preschool front doors proceed to outdoor classroom gate, then drop off or pick up children via the back classroom doors.
2- Gate Supervision: The elementary outside gate will be actively supervised by a designated staff member to allow entering the parents of preschool children only.
3- Installation of alarms: Alarms will be installed on the front doors of Classroom K1 and Classroom K2 to alert staff of any unauthorized door openings.
4- Re-training staff- Staff training regarding 100 percent supervision of children at all times.

During the meeting they stated they installed the alarm devices on both doors on 6/5/2025. They provided pictures of installation. They agreed to send a short video to prove the operation of the devices by the due date of 6/13/2025. They also confirmed that the devices would be checked for batteries by staff. They also stated they went over the supervision training in their meeting with staff on 5/29/2025.

The following were discussed in this meeting:

1- The facility representatives were advised that the facility must be in compliance at all times.
2- Increased unannounced visits to the facility will be conducted by the Department to monitor compliance.
3- The facility representatives were informed that if subsequent repeated violations are cited in the future and the Department determines that the facility has violated the law/regulations or is inadequately implementing the approved plans to remedy the facility's noncompliance, the Department, in its discretion, will seek formal legal action.

Upon receipt of this report pertaining to a meeting conducted by a local Licensing Agency in which issues of were discussed, the licensee must: 1) Provide a copy of this report to the parent/guardian of children currently enrolled by the next business day or immediately upon return. 2) A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). 3) Obtain signature and date from the child's parent/guardian on the Acknowledgement of Receipt of Licensing Reports LIC 9224. 4) Keep a record immediately upon receipt of the completed and signed LIC 9224 acknowledging receipt of this report in the child's file.

The licensee was informed to provide a copy of this report and LIC 9224 (Acknowledgement form) to the parents/guardian of children in care. A copy of LIC 9224 was provided to licensee.


Exit interview conducted with the Oak View Preschool representatives , Luisa Tanzil (Early Learning Supervisor), and Elaine Burney, Coordinator of Early Learning Program who are in agreement with the above. A copy of this report was provided to Oak View Preschool Representatives.
