Regarding the allegation (3), “Staff leaves daycare child in highchair for extended periods of time.” During the visit on 10/23/25, from 3:30 p.m. to 4:15 p.m., an observation was conducted in the classroom. In the toddler classroom, the LPA observed five (5) toddlers in highchairs eating snacks with two (2) staff members nearby.
The LPA observed two (2) of the five (5) toddlers using sign language to say “all done” to signal to the staff. One (1) of the two (2) staff members near the child responded with the sign language “you are done” and then allowed the child to get up. During staff interviews, all five (5) staff members did not disclose any information related to the allegation. All five (5) interviewed parents did not disclose any information related to the allegation.
Regarding allegation (4), “Staff leaves day care children soiled for extended periods of time.” During the visit on 10/23/25, around 12:00 noon, the LPA observed that some infants' cubbies contained dirty clothes wrapped in plastic bags and placed inside the cubbies. During record review, LPA obtained and reviewed the infants' daily logs. In each daily log, there are records of diaper changes, including the time and specific notes indicating whether the diaper was dry, wet, or has a bowel movement (BM), along with the check times, which occur every half hour. If a child gets wet or soiled, the teacher documents the time and notifies the appropriate staff. During staff interviews, all five (5) staff members did not disclose any information related to the allegation. All five (5) interviewed parents did not disclose any information related to the allegation.
Regarding allegation (5), “Staff does not provide adequate meal service.” During the visit on 10/23/25, from 3:30 p.m. to 4:15 p.m., an observation was conducted in the classroom. In the toddler classroom, the LPA observed five (5) toddlers in highchairs eating snacks with two (2) staff members nearby. The LPA observed that the five children were sitting in highchairs and using sign language to ask for more food. The teacher nearby repeated the foods the children requested and served them accordingly. During staff interviews, all five (5) staff members did not disclose any information related to the allegation. All five (5) interviewed parents did not disclose any information related to the allegation.
