During the investigation, LPA conducted facility visits on 6/11/2026. LPA Trinh interviewed seven (7) staff members, five (5) children; two (2) of whom are qualified, three (3) of whom are not qualified, and three (3) parents. LPA Trinh also reviewed records, including the LIC 500 Personnel Report, the LIC 9040 Children’s Roster, and all teacher transcripts. The documentation includes an invoice-receipt for cleaning services from April 2026 to June 2026 from R&R Cleaning (Ramon Ruiz/Robert Ruiz). Additionally, invoice receipts from South Coast Hygiene Services (April 2026 to June 2026), purchase receipts from Costco for Lysol disinfection wipes, and Lysol spray from April 2026 – June 2026.
Regarding the allegation (1) “Staff do not ensure the facility is free of pests or insects.” During visits on 6/11/2026 and today, 07/20/2026. The LPA observed pest traps attached to the electrical outlets in each classroom and obtained photographs. The LPA interviewed seven (7) staff members, all of whom stated that they had placed a few roach traps outside near the playground around the end of April or beginning of May, following the demolition of the adjacent building. Staff noted that since the director placed pest traps throughout the classrooms, they have not seen any insects or pests inside/outside the facility. All staff members stated that after the children leave in the afternoon, a cleaning crew comes to the facility three to four times a week. Additionally, the LPA interviewed two (2) children, both of whom stated they had not witnessed any insects or pests in the facility. All three (3) interviewed parents did not disclose any information related to the allegation.
Regarding allegation (2),” Unqualified teachers.” During the visit on 6/11/2026, the LPA interviewed seven (7) staff members, who confirmed that all classroom teachers—excluding the teacher's aide—hold degrees and are fully qualified. The teacher aides denied that they have worked alone with the children in care. They stated that there is always a fully qualified teacher working with them. During the record review, the LPA verified that all classroom teachers meet the full qualification requirements. All three (3) interviewed parents did not disclose any information related to the allegation.
