Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced complaint visit to initiate an investigation into the above allegation. LPA was greeted and granted entry into the facility and explained the reason for the visit.
During the visit, LPA toured the facility and interviewed staff and resident as well as reviewed and obtained pertinent documentation such as work orders. Regarding the allegation that facility staff are not keeping resident rooms at a comfortable temperature, the investigation revealed the following: Resident 1 (R1) indicated facility temperatures are not comfortable. LPA observed temperature in resident's room is 76 degrees F and dining room/ hallway temperatures are 75 degrees F. Facility provided an oscillating fan to resident for additional cooling. Resident is observed to be wearing a shirt, long sleeve hoodie as well as a puffer jacket while stating the temperatures are cold in the dining room and the many layers are needed. LPA reviewed facility documentation indicating multiple visits to resident's room by Maintenance to observe and/or address the temperatures in the room. CONTINUED ON LIC 9099C DATED 07/10/2025.
