Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA was greeted and granted entry by Staff Richard Vargas. LPA met with Administrator (AD) Elizabeth Calvo and discussed the purpose of the inspection

During the inspection, LPA and AD conducted a tour of the facility, common areas, bathrooms, storage rooms and observed the following:

This is a one-story industrial complex with one main room, two bathrooms, a storage closet, a resting room for clients, staff lounge and one staff office. LPA observed staff and clients engaging in activities such as puzzles, board games, and coloring. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested at 115.5 F degrees.

LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted in various bulletins boards located throughout the facility. Food menu was also posted and visible. Fire extinguisher was observed to be fully charged. Facility was issued Fire Code Permit by Anaheim Fire and Rescue in July of 2023. Disaster drill log indicated that last drill was conducted on 4/05/23. All and any toxic chemicals, cleaning solutions, and disinfectants are inaccessible to clients. The facility does not manage client medications and LPA did not observe any medications on site. The first aid kit has all the required elements. LPA reviewed seven client files and four staff files. LPA interviewed seven clients and four staff.

Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report was left at the facility.
