32 | injuries from 02/2019-04/2019 and 07/2021-07/2022. Home Health was providing wound care for pressure injury on the buttocks as well as necrotic toes. Depending on the month, R1 was being seen by Home Health 1-4 times a month for wound care. Resident is diabetic, an amputee, and on renal dialysis and had been since admission. Per witnesses, R1 is non-compliant with care. Facility and Home Health indicate conversations with family regarding a higher level of care but was met with resistance. Due to conflicting information, LPA is unable to corroborate the allegations. Therefore, the allegations are deemed unsubstantiated, meaning that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. An exit interview was conducted and a copy of this report was provided. |
