25 | Licensing Program Analyst (LPA) Michael Tea arrived on September 24, 2026 for an unannounced visit to follow up on a substantiated complaint investigation delivered on October 31, 2025. LPA Michael Tea met with Executive Director (ED) John Goodwin and Health and Wellness Director (HWD) Putri Tarigan and reviewed the report.
On October 31, 2025, the Department concluded a complaint investigation regarding the following allegation: Resident sustained multiple pressure injuries due to neglect.
The allegation was substantiated, and the licensee was cited under California Code of Regulations § 87464(f)(1) Basic Service.
At the time of the office visit on October 31, 2025, an immediate civil penalty of $500 was issued. Due to the fact that this was a repeated violation, an additional $500 was levied. The licensee was informed that an additional civil penalty might be assessed based on Health and Safety Code §1569.49(f).
The Department has concluded an analysis and has determined that a civil penalty is warranted for serious bodily injury. The Welfare and Institutions Code Section 15610.67 defines serious bodily injury as “an injury involving extreme physical pain, substantial risk of death, or protracted loss or impairment of a function of a bodily member, organ, or of mental faculty, or physical rehabilitation.” The findings and analysis of the documents obtained revealed that the resident was discovered on their bedroom floor by staff after being unaccounted for since the previous evening. The resident was admitted to hospital and diagnosed with deep pressure ulcer of left cheek after being on the floor for approximately 24 hours.
Today September 24, 2026, the Department will be issuing a civil penalty per Health and Safety Code § 1569.49(f) for a violation that the Department constitutes as serious bodily injury in the amount of $10,000.
