32 | Regarding the allegation, resident sustained unexplained bruising while in care, the following has been concluded: It was alleged that Client #1 (C1) sustained unexplained bruising while in care. The Department was unable to conduct an interview with C1 for this complaint due to C1 moving out of the facility on November 4, 2022. The Department attempted to speak with C1's conservator, however, they declined to be interviewed. The Department conducted six staff interviews. Five out of the six staff interviewed denied the allegation and stated that they were unaware of C1 developing any unexplained bruising. The staff interview stated that the bruising could have been a result of a fall C1 sustained while at the facility. However, the staff also stated that each fall was documented, reported to the appropriate parties, and that medical attention was sought for C1 when needed. One of the six staff interviewed was unable to provide any information for this allegation due to the staff not being an employee during the time.
Regarding the allegation, staff did not address resident's change in condition while in care, the following has been concluded: It was alleged that staff did not address C1 change in condition while in care. The Department was unable to conduct an interview with C1 for this complaint due to C1 moving out of the facility on November 4, 2022. The Department conducted six staff interviews. One of the six staff interviewed was unable to provide any information for this allegation due to the staff not being an employee during the time. However, five out of the six staff interviewed denied the allegation and stated that C1's change of condition was addressed appropriately. The staff interviewed confirmed that C1 did have a change of condition while in care, but, these changes were documented, and reported to C1's conservator. The staff interviewed stated that the changes in C1's condition made him no longer appropriate for the facility, and as a result, C1's conservator was provided with a 30-day eviction notice. The Department attempted to speak with C1's conservator, however, they declined to be interviewed. The Department reviewed C1's file. The Department observed that C1's conservator was provided with 30-day eviction notices on January 18, 2021, June 9, 2021, June 27, 2021, July 9, 2021, September 20, 2021, and October 4, 2022. The Department observed that each of the 30-day eviction notices that were provided to C1's conservator cited that C1's needs could no longer be met by the facility, due to the changes in his condition.

The Department was unable to conduct an interview with C1 for this complaint due to C1 moving out of the facility on November 4, 2022. The Department conducted six staff interviews.
