On 5/23/24 Licensing Program Analyst (LPA) Mario Leon made an unannounced, required annual visit, to the above-mentioned facility and met with Program Director Gloria Patricia Ball (S1) and the purpose of the visit was explained.
The facility is licensed for a capacity of twenty-seven (27) non-ambulatory, developmentally disabled, clients with restricted health conditions. The Program Director stated that the facility has thirty-seven (37) clients currently enrolled in the program which consist of the following: eighteen (18) clients in the morning, of which two (2) are remote, and nineteen (19) clients in the afternoon of which three (3) are remote. All clients are Harbor Regional Center clients. The staff to client ratio is one (1) staff to every 3 clients. Program Director stated that five (5) of the clients have restricted health care conditions, all of which the above-mentioned facility is able to handle. LPA observed paperwork for each client, within clients' folders. The last disaster drill (fire) was conducted on 04/11/24 and a Disaster Plan (LIC610D) was on file. The last inspection held by the fire department was on 03/30/24. The program is able to share transportation vans, for community outings, with the neighboring Adult Residential Facility, CN Home #4.
As a part of today's inspection, LPA reviewed five (5) client records and five (5) staff records, of which 1 was not available for inspection, see LIC9102TV. LPA inspected the physical plant. The facility is an Adult Day Program, located in a residential neighborhood consisting of the following: a gated parking lot (drop off/pick up area), lobby area, four activity rooms, Kitchen, Client/Staff storage area (cubbies/lockers), 1 directors office, 1 nurse office, two (2) bathrooms (men’s and women’s), gated outside shaded area, and a garden area in the Eastern side (rear) of the parking lot. Interior and exterior walkways and all exits were clear of hazards and debris.
LPA conducted a tour of the physical plant and observed the following: walls and floors were in good condition, adequate lighting was provided and 2 fire extinguishers were properly charged, as of 04/29/24.

LPA observed plenty of storage space and observed cleaning solutions were not properly locked, see LIC9102TV. The restrooms were clean and water temperature was measured between 105.3 degrees Fahrenheit and 110.3 degrees Fahrenheit throughout the facility, which are within Title 22 regulations.
The day program does not provide lunch nor snacks to clients, yet clients do conduct kitchen activities one time per week. The first aid kit was available and fully stocked. The above-mentioned facility does not administer medications, as clients are provided their medication either before or after their session at the facility.

During today’s visit LPA provided 2 technical violation notes, see LIC9102TV.

An exit interview was conducted with Program Director, Gloria Patricia Ball, and a copy of this report and the technical violations were provided.
