Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required 10-day visit. LPA met with Administrator Marjan Arastoo. The investigation into the allegation revealed the following. It was alleged that Client 1 (C1) was not assisted with grooming, specifically with applying body lotion. According to the preplacement appraisal, and the most recent appraisal /needs and services plan, C1 can accomplish grooming and hygiene on his own and only needs reminding to complete grooming and hygiene. 6 out of 6 staff interviewed reported that C1 has to be reminded to conduct daily grooming/hygiene everyday and C1 is reluctant but does perform daily grooming/hygiene including putting on body lotion. LPA interviewed C1 who reported they practice grooming and hygiene unassisted including putting on body lotion. It was alleged that staff said they do not have to put lotion on C1. Staff denied this allegation. None of C1's medications are handled by C1 or the facility. LPA reviewed facility medication administration records and C1's client file and verified that the facility and C1 do not handle any of C1's medication. C1 is only prescribed on medication (not handled by C1 or the facility) and it is not any type of body lotion. None of the evidence gathered through record review or interviews corroborates the allegation.

Based on the evidence, the allegation, staff do not assist resident with grooming is deemed, unsubstantiated, although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted and a copy of the report provided.
