Licensing Program Analyst (LPA) Celine De Perio conducted an unannounced 10-day visit to the facility for the complaint received on 7/7/23 and to deliver the findings. LPA De Perio explained the purpose of today's visit, was greeted, and granted entry by facility administrator (AD) Marjan Arastoo.

During the investigation, LPA De Perio toured the physical plant of the facility, conducted interviews, and requested copies of pertinent records reviewed.

It was alleged that staff did not ensure that residents have hot water while in care. A total of 7 interviews were conducted. 3 interviews were conducted with staff, and 3 out of the 3 interviews conducted with staff did not corroborate with the allegation by stating that the facility has a central water heater and "it covers the entire first floor, second floor, bathrooms and kitchen".

1 out of the 3 interviews conducted with staff also added "there are some people here that want it very, very hot but we can't do that because it would burn their skin". 4 interviews were conducted with clients, of which all 4 interviews stated that the facility does have hot water. 1 client interview stated "there is hot water here, but I want it boiling hot because it feels good". Another client interview stated "it takes a few minutes for it to warm up". During the physical plant of the facility, LPA De Perio tested the water of 4 client bathrooms located in their rooms, 2 showers located on the first floor, and 1 public bathroom also located on the first floor. Water temperatures in the bathrooms and showers were measured to be within the range of 108.3 and 111.9 degrees Fahrenheit. LPA De Perio reviewed documents such as the facility staff schedule, cleaning schedule, and the water temperature log. Per the water temperature log, it was observed that the waters were measured to range from 107.5 to 115.0 degrees Fahrenheit.

It was alleged that facility restrooms are dirty. A total of 7 interviews were conducted. 3 interviews were conducted with staff, and 3 out of the 3 interviews conducted with staff did not corroborate with the allegation. 1 out of the 3 interviews conducted with staff stated "sometimes the clients will dirty their bathrooms but we still clean it". 4 interviews were conducted with clients, of which 3 of the clients stated "the cleaning is fine" and "someone cleans". 1 out of 4 interviews conducted with clients stated "it does get dirty but there is a lady who cleans and she does a good job". LPA De Perio conducted a tour of the facility and during the tour, LPA De Perio observed that there was a staff on duty cleaning the two facility showers located on the first floor. LPA De Perio also toured 1 public facility restroom and 4 client bathrooms and all 5 restrooms were observed to be clean. LPA De Perio reviewed documents such as the facility staff schedule, cleaning schedule, and house rules.

Based on LPA’s interviews which were conducted, review of documents obtained, and observations, LPA is unable to ascertain if the allegations occurred as reported. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violations occurred; therefore, this allegation is deemed UNSUBSTANTIATED.


An exit interview was conducted with AD Arastoo. A copy of this report was explained.
