On July 15, 2026, the California Department of Social Services/Community Care Licensing (CDSS/CCL) Licensing Program Analyst (LPA) Ernand Dabuet conducted an initial unannounced complaint visit. Jassmond Johnson, the Administrator, greeted the LPA and explained that the purpose of the visit was to investigate the allegation mentioned above.

The investigation involved collecting documents and inspecting the facility. A review was conducted of the Personnel Report LIC 500 (dated March 2, 2026), the Resident Roster (dated February 10, 2023), the American Structural Pest Control Service Record (dated March 10, 2026), and other pertinent records related to the complaint. Interviews were conducted with Clients #1 and #2, Staff #1 through Staff #3, and Witness #1.

INVESTIGATION REVEALED THE FOLLOWING:

Allegation: Staff do not prevent long-term rodent infestation.

On July 15, 2026, between 9:30 AM and 10:45 AM, the Department examined the facility's interior, which included the kitchen, pantry, dining room, living area, client bedrooms, garage, and exterior perimeter. The inspection revealed no evidence of rodent activity, such as droppings, nesting materials, entry points, odors, or damaged food packaging. The exterior inspection also showed no holes or gaps, vegetation, standing water, trash, or debris.

On July 15, 2026, between 9:45 AM to 11:00 AM, the Department interviewed staff members identified as Staff #1 through Staff #3 (S1-S3). Three (3) out of the three (3), none could corroborate the claim of a rodent issue. Staff #1 (S1) mentioned that while there have been occasional sightings of rodents over the past couple of years, particularly during weather changes, Staff #2 (S2) reported observing rodents in the facility as recently as July 14, 2026. Staff #1 (S1) stated that rodent activity had been reported on July 14 and that a request for pest control service had been submitted for July 15. Staff #3 (S3) indicated that rodent activity has fluctuated over the past couple of years, but (S3) could not recall the most recent sighting. All staff members confirmed that the facility is proactive in addressing this issue by maintaining a pest control services agreement and using four rodent bait station boxes and several mousetraps throughout the common placed by the pest control company.

On July 15, 2026, between 9:50 AM and 10:15 AM, the Department interviewed Clients #1 and #2 (C1 and C2). Two (2) out of the two (2) clients reported no concerns and indicated they had not seen any rodents in common areas or their private bedrooms. Clients #3 (C3) and #4 (C4) were unavailable for interview as they were out in the community during the visit.

On July 15, 2026, between 01:00 PM and 1:20 PM, the Department interviewed witness identified as Witness #1 (W1). (W1) confirmed that the facility currently has an active service agreement for monthly pest control and weekly rodent control. (W1) mentioned that the facility has implemented proactive measures to address ongoing rodent issues. Additionally, the company is restricted to providing only Rodent Bait Stations in compliance with state licensing regulations, which prohibit the use of toxic chemicals on the premises.

The Department reviewed records from American Structural Pest Control West (dated March 10, 2026), which indicated that the facility had received rodent pest control treatment on several occasions in 2026, specifically on January 13, January 22, February 3, February 11, February 20, June 29 and July 15. Records confirmed that these services were performed in accordance with standard pest control procedures. Monitoring devices, including rodent bait boxes and mousetraps, remain in place to assess rodent activity and support ongoing control efforts. The Department observed these monitoring devices in the common areas of the facility.

Based on the information gathered, there is insufficient evidence to support the allegation mentioned above.

Based on the information collected from the facility inspection, observations, interviews, and records analysis, the Department found no evidence to support the above allegation. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred. Therefore, the allegation is Unsubstantiated.
