25 | On July 21, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Ferdinand Carino, House Manager and Mary Cruz, Administrator and explained the purpose of today's visit. The facility is licensed to operate for (18) mentally disabled adults ages 18-59.
The facility is a one-story structure located in a commercial neighborhood. It consists of the following: (11) client's rooms and (1) restroom, (1) activity room, (1) dining area, (1) kitchen, (1) staff room, (1) staff restroom, and covered gazebo.
LPA toured the physical plant with Carino. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in satisfactory condition, adequate lighting was provided, and storage for the client's personal belongings was observed. The rooms were inspected. Bathrooms were operational, with water temperatures measured at 105.0 °F. A comfortable temperature was maintained in the facility at 78 degrees F.
LPA observed the facility to be sanitary and furnished during the visit. Storage areas for personal hygiene items, cleaning supplies, toxins, and sharp objects were kept out of clients' reach. The kitchen was inspected, and sufficient perishable and non-perishable food was available and maintained properly. Fire extinguishers were charged, and smoke detectors and carbon monoxide detectors were operable. A review of the Fire Drill conducted on June 7, 2026, at 1:00 PM. The facility has a current Emergency and Disaster Plan reviewed and updated May 28, 2024.
