Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required 10-day visit to begin the investigation into the allegation listed above. LPA met with staff and explained the reason for the visit. The investigation into the allegation revealed the following. LPA interviewed staff members present at the facility during the visit. LPA attempted to interview the 5 clients presents. 2 of the 5 clients are non-verbal and did not respond to the LPA. 3 out of 5 clients are verbal but have a limited vocabulary and did not answer any of the LPA's questions. It was reported that facility staff lock clients in their room. During the visit LPA asked to see Client 1's (C1) room and Staff 1 (S1) produced a bedroom door key (resembles an allen wrench) that can open any bedroom door that is locked in the facility, S1 opened the locked door with the key. 3 out of 3 staff reported that they lock C1's room because of Client 5's (C5) behavior. 3 out of 3 staff reported that they lock C1's room so C5 stays out of C1's room and doesn't disturb C1. All Clients have private rooms and each door has a lock. Once the bedroom door knob is locked it can be opened from inside the room just by turning the knob but from the outside it can only be opened by using the bedroom door key.

LPA verified with staff that C1 does not lock their own bedroom door. Staff verified that C1 is not in any type of danger that their door needs to be locked. Based on the evidence gathered the preponderance of evidence standard has been met, therefore the allegation is substantiated. Deficiencies are being cited per Title 22, Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of the report along with appeal rights was provided.
