Licensing Program Analyst (LPA) Galarza conducted an unannounced Required- 1 year visit. The purpose of the visit was explained to Administrator Brian Buenviaj. The Adult Residential Facility (ARF) is licensed to serve 56 ambulatory and 6 non-ambulatory mentally disabled residents ages 18-59. The facility provides transportation services.

The following were observed/inspected:



Infection Control: The facility has an Infection Control Plan.

Physical Plant/Environment Safety: The facility is composed of 2 single-story buildings and one (1) two-story building, with a total of 30 resident rooms with shared bathrooms, dining hall, medication room, kitchen, laundry room, TV area, 2 offices, recreation room, and outdoor covered patio areas. The kitchen and dining room are located in the middle of the property in a separate building. The facility is equipped with a sprinkler system and smoke detectors. There are fire extinguishers in all hallways and kitchen. Cleaning supplies and toxic substances are inaccessible to clients. Water temperature readings measured between the required 105 - 120 degrees Fahrenheit. The facility maintains emergency food supply and water. The facility has a 1st Aid Kit/Manual consisting of thermometer, tweezers, scissors, antiseptic, bandages, gauze. Hygiene supplies are provided. Exit doors are free of any obstruction and there are no pools or large bodies of water. However, many of the interior facility resident rooms and common areas have deferred maintenance i.e. wall and ceiling repairs are needed and resident rooms need painting.

Operational Requirements: Care and supervision to meet the clients needs was observed. Special equipment is used by some residents. The facility handles resident's monies. The Surety Bond expires 7/15/25. The last fire inspection was conducted on 11/29/2024 by LA County Fire Department.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Staffing: A total of 13 staff members provide care and supervision to the clients.

Personnel Records/Staff Training: Seven (7) staff files were reviewed for criminal background clearance and training. Personnel records have health/TB screenings, 1st Aid/CPR training, and training records. None of the files had proof of continuous staff training. Staff (S2) main cook does not have a food handling training and some staff files are missing SOC 341 training. Administrator certificate expires 10/3/25.

Client Rights/Information: Personal rights were only observed in files and not posted in public areas.

Client Records/Incident Reports: Eight (8) resident files were reviewed. The majority of files have documents i.e., Physician's Reports that have not been updated since admission date, which in some cases is over 10 years. Licensee is to ensure records are reviewed and updated as necessary for any changes in condition.

Food Service: Food supply is stored in the kitchen and pantry areas consisting of: 2-day perishables, 7-day non-perishables, and emergency food supplies. Kitchen, food preparation area, and storage areas were observed to be clean and sanitary. The facility has emergency water and food supply and snacks on site. Some residents require modified diets.

Health Related Services: Residents are assisted with self administration of prescription and non-prescription medications. Three (3) 30-day supply of centrally stored resident medications/records were reviewed and are given according to Physician directions.

Incident Medical and Dental: Resident files include Physician Reports and health screenings.

Disaster Preparedness, and Emergency Intervention: The facility has an updated Emergency Disaster Plan containing emergency evacuation information. An emergency drill was conducted on 3/26/25, within the last 6 months as required.

Emergency Intervention: No manual restraints or seclusion are used with clients in care.

Per Title 22, deficiencies were cited.
