Licensing Program Analyst (LPA) Lydia Martinez made an unannounced visit to the facility for the purpose of conducting a Required 1 Year inspection. LPA was greeted and granted entry by Licensee/Administrator Janet Tounzen and reason for visit was explained. Assistant Director Magda Vazquez arrived shortly after.

During the inspection LPA and Assistant Director Vazquez conducted a tour of the inside and outside of the facility and observed the following:

Facility is a single-story commercial building and is shared with a church. Facility consists of three activity rooms, three bathrooms, one staff office, one staff lounge, and an enclosed shaded sitting area located at the back of the building. LPA observed one staff to four client ratios. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Hot water temperature is within regulatory requirements. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Clients bring lunch from home and day program provides snacks and water. Sharps, all and any toxic chemicals, cleaning solutions, and disinfectants are inaccessible to clients. Medication cabinet is located in the staff office and was observed to be locked. Facility manages medication for one client only. Clients and staff files are locked in a file cabinet in the Director's office. Six clients and three staff files were reviewed. All staff have current First Aid/CPR certifications on file. LPA observed clients interacting and participating in recreational activities, playing instruments and listening to music. Several activities are available and offered at the facility, such as, board games, reading books, music instruments, arts & craft, and exercising videos. Facility offers and teaches clients on social/life skills, such as, courtesy, respect for others, and mannerisms. The facility conducts disaster & fire drills every 4 months and the last fire drill was conducted on 10/17/2024. Fire extinguishers were serviced on 10/18/2024.

Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. This report was discussed with Assistant Director Magda and a copy was sent to email on file.
