On 3/20/2024 LPA conducted interviews at the facility with the Alleged Victim (AV) who corroborated the allegation. Interviews were conducted with Clients 2 and 3 who did not corroborate the allegation. LPA attempted to interview Clients 1, 4 and 5 and was unable to due to clients being non-verbal.
LPA interviewed PD and facility staff. No disclosures about the allegation were made.
LPA requested copies of the following client files: Admission Agreements, Client Development Evaluation Reports, Individual Program Plans, Individual Service Plans and physician's reports. LPA requested copies of the following staff files: personnel reports, employee acknowledgments regarding body checks, incident reporting and abuse reporting. Electronic copies of the files were provided to the LPA via email during the inspection.
Due to conflicting information received during interviews conducted, LPA was unable to determine if staff handled a program client inappropriately. Based on interviews conducted and records reviewed there is insufficient evidence to support the allegation. Although the allegation(s) may have happened or is valid; there is not a preponderance of evidence to prove that the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
An exit interview was conducted, and this report was reviewed with Program Director Timothy Thirtle. A copy of this LIC-9099 was provided to the facility.
