Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to investigate a complaint received in the Regional Office. LPA was greeted and granted entry by staff at 1pm and explained the purpose of the visit. LPA met with Administrators Dante Benedicto and Marevic Painter.

The following documents were obtained for Client #3 (C3): Face sheet, Regional Center Admissions Agreement, Medical Assessment dated May 7, 2025 and monthly physical and psychologist assessments for June 2026, Medication Administration Record and Physician's Orders for July 2026, Individualized Program Plan (IPP) and Client Development and Evaluation Report (CDER) dated October 9, 2025.

It was alleged Client's medications were not given as prescribed. LPA reviewed three of three client medications. Per medication audit, three of three client medications are being given as prescribed. LPA reviewed the Medical Assessment from May 7, 2025 and current monthly medical assessments on June 2 and June 3, 2026. Medications are reviewed by the Primary Care Provider (PCP) with staff and client.

LPA reviewed Medication Administration Records (MAR) for all clients. On July 1 2026 It was observed that the medications for Client #3 (C3) were initialed for 9pm by signed by staff prior to 5pm. Interviews of three of three staff revealed that the staff member on this date did initial on the MAR early but did not give the medication until the designated times of 7pm and 9pm. Medications remained in the medication bubble pack and are not pre-poured. Staff recall being questioned about this but did not explain that the medications were still in the bubble pack and confirmed they initialled the MAR early.

Three of three staff shared that C3 asks to receive medication early daily. Staff stated medications are only given with meals at 5pm and on the times notated on the MAR. Medication training was provided for staff in January 2026. Administrator Benedicto will email LPA medication training documentation. A refresher course will be conducted for staff to not sign the MAR until medications are actually given.

LPA interviewed two of three clients regarding medications. Two of three clients reported they are given medications and that they do not have any issues with medication administration. Client interviews confirmed that one of the clients will always ask for medication early to go to bed; but stated staff do not provide the medications until the designated time.

Based on LPA's document review, interviews and observations, the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred. Thus the allegation that Client's medications were not given as prescribed is Unsubstantiated. An exit interview was conducted with Administrators Dante Benedicto and Marevic Painter and a copy of this report and LIC 811 were provided to the facility.
