Licensing Program Analyst (LPA) Kimberly Lyman made an unannounced visit to the facility to deliver findings on the above allegation. LPA was greeted and granted entry into the facility by Administrator Patty Osuna and explained the reason for the visit. Care Coordinator Alexis Jones was present as well.

During the course of the investigation, LPA toured the facility, interviewed staff and witnesses as well as reviewed and obtained pertinent documentation such as physician report. Regarding the allegation that staff did not feed resident while in care, the investigation revealed the following: Two out of two staff interviewed indicate Resident 1 (R1) is being fed at the facility. Staff members indicate R1 prefers to eat in the resident's room and requests tray service which staff state they have been providing. The resident indicated to staff that the resident has not had an appetite since recovering from covid due to lack of taste. Resident confirmed with LPA the receipt of tray meals most of the time as well as a preference to eat in the resident's room. Facility staff indicate a tray service fee but R1 has not been charged the fee to date. Facility staff explained the importance of residents going CONT ON LIC 9099C DATED 09/08/2022

Licensing Program Analyst (LPA) Kimberly Lyman made an unannounced visit to the facility to deliver findings on the above allegation. LPA was greeted and granted entry into the facility by Administrator Patty Osuna and explained the reason for the visit. Care Coordinator Alexis Jones was present as well.

During the course of the investigation, LPA toured the facility, interviewed staff and witnesses as well as reviewed and obtained pertinent documentation such as physician report. Regarding the allegation that staff are not following physician's orders, the investigation revealed the following: R1's preference is to eat in the resident's room as the resident is recovering from multiple covid infections and prefers to eat alone. Two out of two staff and resident indicate there has not been an order from physician for resident to eat in room. Facility is providing periodic tray service without an order. Resident has approached physician for an order but order had not been received at time of complaint. Therefore the allegation is deemed UNFOUNDED, meaning the allegation was false, could not have happened and/or is without a reasonable basis. Exit interview conducted and a copy of this report was left at the facility.

to the dining room for socialization and moved the resident's room closer to the dining room for easier access. Due to conflicting information, LPA is unable to corroborate the allegation. Therefore, the allegation is deemed unsubstantiated, meaning that although the allegation may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted with and a copy of this report was provided to facility.
