25 | On this day Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced visit for the purpose of conducting a required annual visit. LPA was greeted and was granted entry into the facility by staff. LPA's temperature was checked upon arrival. Facility has Temperature check in station with hand sanitizer inside entry hallway. LPA Tirre met with Administrator Shelly Martinez and explained the reason for the visit.
During the visit LPA toured the facility with Caregiver and Administrator, Facility is a 4 bedroom (3 Client rooms and 1 staff room) and 2 bathroom single story home. There are 6 Clients in care. During time of visit there were no clients present, clients went to Day program. LPA observed facility has required Department postings. LPA toured all clients rooms, all rooms where within regulations. All restrooms observed contained working water basin, soap, toilet paper, and paper towels. Proper hand washing signs were posted inside restrooms.
Facility has supplies of PPE. Facility has ample food supply. LPA observed facility has emergency food and water supply. Facility has secure locations for sharps, Facility has secure location for Toxins and hazardous materials. Facility has evacuation plan posted on wall. LPA observed Administrator certificate expiring on 04/03/24. Facility has Mitigation and Infection Control plans inside binder readily available. Facility has 1 fire extinguisher fully charged. Facility has a secured location for Client medication and files. Facility has 30 days supply of medications for clients. LPA reviewed Clients files during visit. Clients emergency contact information and physicians reports are current. 6 out of 6 files were observed. Facility has designated area for visitors.
No deficiencies noted during todays visit. An exit interview was conducted with Administrator and a copy of report was left at facility. |
