25 | On 07/10/2020, Licensing Program Analyst (LPA) Lydia Martinez contacted the facility via telephone to commence a Case Management virtual visit via FaceTime due to COVID-19 and pre-cautionary measures. Facility is requesting a floor plan change (moving Memory Care from 3rd floor to 1st floor). Facility currently has 44 residents in Memory Care on the 3rd floor. A Fire Clearance was granted on 05/12/2020. LPA Martinez discussed the purpose of the call with Administrator Hope Pak.
LPA Martinez toured the interior and exterior of Memory Care which is located on the 1st floor via Face Time with Administrator Pak. The tour included resident rooms, common areas, dining hall, and outside features. There are 29 rooms in Memory Care, delayed egress exits were tested and observed to be operational. During the tour the following was observed: Room 101 will be an office; 102 thru 107 and 109 will be for residents, 108 and 110 are connected with a walk-in shower, therefore, it will be used as a common shower. 111 thru 119 are resident rooms, 120 and 122 will be model rooms, 121 will be a resident room, 123 a resident room, 124 will be used for storage/future manager's office and will be remained locked at all times as this room has sliding doors that lead to the back building parking lot. Room 125 thru 127 will be resident rooms. Room 128/Linen room has 3 non-working washers and a link fence door inside the room. Administrator Hope has agreed to throw out non-working appliances and remove chain link fence on or before 07/23/2020 and prior to moving resident to the 1st floor. 129 will be a resident room, 131 will be a staff office, 133 will be a storage room and Administrator Hope has agreed to install an Alarm Key Lock. Memory Care unit has its' own dining room. Meals will be provided from the main kitchen of the facility. Room in dining area that connects to Maintenance shall be kept locked at all times. Per Administrator, caregivers will not have access to this room. Administrator stated shrubs on the outside will be cleaned prior to moving the residents. Administrator has agreed to submit pictures as proof of correction by 07/23/2020 and prior to moving residents in.
An exit interview was conducted with Administrator Hope Pak via FaceTime and a copy of this report was provided via email. Administrator agrees to sign report and email back to LPA Martinez., |
