25 | Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to complete the Required One-Year Inspection. LPA explained the purpose of the visit and was greeted by Licensee Emmanual Villacorta, who granted entry. At the time of the visit, there were three clients in care and all clients were present.
The facility is a one-story home with five bedrooms, three bathrooms, a kitchen, dining room, living room, backyard, and an attached one-car garage.During the inspection, LPA and the Licensee tested the smoke detectors and carbon monoxide alarms in the common areas and client bedrooms, and all were operational. Client bedrooms were observed to have the required furniture, bed linens, and adequate closet and drawer space to accommodate each client comfortably. LPA observed that sharps and knives were secured in a locked location in the kitchen and were inaccessible to clients in care. Restrooms, toilets, and water faucets were observed to be operational. Grab bars were secure, showers were observed to be free of mold and mildew, and the water temperature measured between 113.9 and 114.6 degrees Fahrenheit. LPA toured the backyard and observed a shaded seating area available for clients’ enjoyment. Emergency exits were checked and observed to be free of obstacles.
Fire extinguishers were observed to be mounted in the dining area, with a last inspection date of January 5, 2026. Upon review of records, the emergency drill log indicated the last drill was conducted on December 5, 2025. Required Department postings were observed to be displayed in the facility.
