Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced visit for the purpose of conducting a required one year infection control annual visit. LPA was greeted, granted entry by staff and explained the reason for the visit. LPA Haley's temperature was check and recorded upon entering the facility.

At 12:15 PM LPA Haley began the tour of the facility, at that time there were 75 residents present. A screening station was observed at the entrance of the facility equipped with a hands free temperature thermometer, log book, and hand sanitizer. LPA observed all residents and visitors being screened upon entering the facility.

The inspection began with checking random resident rooms on the lower level and recording hot water temperatures. All resident rooms that were inspected were clean, well organized, and had all the necessary requirements: night stand, chair, lamps/lights and storage space. Bathroom water temperatures were recorded in the range of 109.5 - 118.9 degrees Fahrenheit.

At 12:25 LPA Haley observed the medication room. All medications were well organized, kept locked, secure and away from the residents in care. There was a brand new first aid kit in the medication room, and equipped with the required elements. While inspecting resident rooms on the lower level of the facility, at 12:31 PM, the call light system was tested and was operational.

Around 12:35 PM LPA Haley observed the kitchen area that was clean and well organized. The facility has a two day supply of perishable food items and seven day supply of nonperishable food items. All knives and sharp objects are properly stored and out of the reach of residents in care.

At 12:40 LPA Haley and staff went up to the second level to inspect random rooms and the memory care unit. All rooms were clean, well organized, and furnished with the necessary items. Room #229 was vacant and is the designated quarantine room. The call light system on the second level was tested and is operational.

During the tour of the second level LPA Haley observed a storage room that was used for storage of various items and had clear walk ways free, of tripping hazards. At 12:46 PM LPA Haley observed the activities room. There was cabinets used to store games and different activities for the residents. There were two patios where residents can step outside the activities room, and enjoy some fresh air.

LPA Haley observed a PPE storage room with plenty of additional PPE supplies and daily PPE kits set up and labeled for employee use. While touring the facility several employees had facility name tags on, which made them easy to identify and check on the facility personnel report. All employees checked were associated and cleared.

At 1:05 LPA Haley toured the outside portion of the facility. The outside area was clean, well organized, and free of clutter and debris. LPA observed several tables equipped with sun shades and chairs. The building is freshly painted and a new facility sign has been placed on the front of the building.

The facility has a brand new alarm system that was recently installed, and brand new patio doors. The sprinkler system was tested on May 16, 2022. The facility records emergency drills, and drills were conducted in January 4, 2022.

There were no bodies of water observed. No deficiencies are being cited during todays visit. An exit interview conducted and a copy of the report was provided to Administrator Jeffrey Po.
