Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced visit for the purpose of conducting a required annual visit. LPA Haley entered the Adult Day program and was greeted and granted entry by staff and explained the reason for the visit.

At 10:10 AM LPA Haley began a tour of the facility with staff. At the entrance of the facility is a large lobby with a reception area that was very well organized. A fully charged fire extinguisher was observed on the floor right next to the conference room/office area. In the conference room/office area is a locked filing cabinet for staff files. The first aid kit with all the required elements is also kept in the staff office. On the other side of the lobby there are two additional rooms. There is a storage room currently being used as a quite space for participants that need quiet time. Next to the storage room, there’s a computer room with 4 desk top computers for the participants to use. In the computer room, client files are stored in a locked filing cabinet.

The day program had a large area called the main hall that was clean and organized. The Main hall is divided into two sections. One section is used for activities, while the other side is mostly open space and used for activities like morning exercising.

The facility has men’s and women’s restroom areas. Both restroom areas for the participants were clean and well organized. The women’s restroom has a shower available for clients to use if needed. Hot water was measured in the range of 114.2 – 114.9 degrees F. All toilets were in good working order. There were no hazardous chemicals stored in the restroom areas.

The kitchen was clean and well organized. The kitchen area is off limits to participants unless participating in one of the classes available to the participants.

No knives or sharp objects are stored in the kitchen. A full charged fire extinguisher was mounted on the wall. Next to the fire extinguisher mounted on the wall was a carbon monoxide detector.

The exterior portion of the facility was clean, with clear walkways free of clutter and debris. There's a shaded patio area with tables and chairs. There's also a gardening area outside available for the participants. The program has four vans for transportation that are clean and well maintained.



LPA Haley observed three fully charged fire extinguisher, and a carbon monoxide detector that tested operational. TAASCO Alarm and Sprinkler Co. Inc tested the fire alarms and sprinkler system May 21, 2024. The last fire drill was conducted August 15, 2024, and are conducted monthly. No bodies of water observed.

7 staff files were reviewed, 7 client files were reviewed, and 4 staff interviews were completed.

No deficiencies are being cited during todays visit. An exit interview conducted, and a copy of the report was provided.
