25 | On June 9, 2025, Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual evaluation at the facility. The Administrator, Heather Ta, granted Mr. Haddadin entry.
The facility is a level-three, single-story building with an approved fire clearance for four ambulatory residents. At the time of the visit, the facility had a census of three clients. One client was present, and two were attending a day program. During the visit, the LPA toured the facility and inspected the physical premises. This inspection included testing all smoke detectors, checking the hot water temperature in both client bathrooms, and verifying the functionality of auditory devices on all exits. The hot water temperature measured between 111.3°F and 111.8°F, and all smoke detectors were operational. The fire extinguisher was fully charged and had been serviced on February 2, 2025. The facility's most recent disaster drill was conducted on February 15, 2025.
The LPA inspected the food supply and confirmed that the facility maintained a minimum of a two-day supply of perishable food and a seven-day supply of non-perishable food. The LPA also observed the medication storage area and reviewed the centrally stored medications, which appear to be administered as prescribed.
A review of two of the four staff files, including training and fingerprint records, was conducted. The LPA also performed a complete review of client records. The client P&I (Personal and Incidental) records were found to be accurate.
Based on the observations made during this visit, the facility appears to be in compliance with the California Code of Regulations, Title 22, Division 6. No deficiencies were cited.
An exit interview was conducted with Administrator Heather Ta, and a copy of this report was provided.
