25 | Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit for the purpose of conducting a required/ annual visit. LPA was greeted and granted entry into the facility by Executive Director Suzanne Nasraty and explained the reason for the visit. Resident Health Services Director Marirose Kendle was present as well.
At 10:16 AM, LPA toured the facility with the management team. There are 536 residents residing in the facility during today's visit, with one on hospice. All residents observed appeared happy and well taken care of. Facility appears clean and sanitary. Facility residents are independent and reside in their own apartments/ homes. LPA observed residents participating in a lecture at the facility. Public restrooms have hand washing signage posted as well as soap/ sanitizer. Facility screens all visitors to the facility at the entrance gate including covid related questions and temperature taking. Facility has covid precaution/ hand washing postings throughout the facility as well as all required department postings. Mitigation plan has been approved. LPA observed ample emergency food and water as well as emergency kits including flashlights, headlights, medicine bags, blankets and collapsable stretchers. LPA observed the facility first aid supplies which contained all required elements. Facility has generators for emergency use. Fire extinguishers are mounted and charged. Facility contracts an outside agency, Dynalectric, for smoke detector testing and the last visit was on 10/05/2021. State Fire Marshall conducted sprinkler testing on 04/01/2022. LPA toured the outside grounds and observed multiple shaded outside visitation areas as well as an indoor/ outdoor pool and recreation areas. LPA observed a gym, movie theater, computer lab and multiple areas for socializing. All recreation areas have proper covid signage and sanitizer present. Indoor visitation areas have proper signage as well. Facility has more than a thirty day supply of all PPE and sanitizer. Facility has a plan for covid testing residents and staff as needed as well as a plan for isolation.
LPA consulted with Administrator regarding the importance of posting the "Let Us No" poster in the entrance of the facility regulation size, 20" X 26."
No deficiencies noted during today's visit. An exit interview was conducted and a copy of this report was left at the facility. |
