Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required 10-day visit to begin the investigation into the allegations listed above. LPA was screened for Covid-19 and granted entry. LPA met with Executive Director Paul Brown. LPA explained the reason for the visit. LPA interviewed staff and residents. The investigation into the allegations revealed the following; It was reported that the Administrator was interfering in the resident council and there meetings. Residents interviewed could not corroborate that allegation. Information received from residents was contradicting, It is unclear during which meeting(s) this occurred. Staff interviewed reported that they have spoken at meetings but then leave so the residents can conduct the meeting with residents only. There is an election for a new resident council president being held at the end of January. It was reported that the facility staff was conducting the new election. Staff reported they were only assisting with posting and creating the election posters for residents. No one interviewed including staff and residents could agree on what exactly led to the upcoming election. All residents interviewed did report they agreed to participate in the new election. The resident council could not produce any minutes documenting the meetings. It is unclear exactly what events led up to the new election.

But all residents interviewed stated they have agreed to participate in a new election and agreed to abide by the results. Based on the evidence gathered through interviews of the staff and residents the allegation, facility is interfering in resident council, is deemed unsubstantiated. Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur. In regards to the allegation staff spoke inappropriately to resident, the investigation revealed the following; staff interviewed including the Administrator were unaware of any staff speaking inappropriately to any residents. 1 out of 5 residents interviewed reported staff could be rude to them but wasn't sure if amounted to inappropriately speaking to them. 4 out of 5 residents reported that they had no issues with staff. Based on the information gathered through interviews the allegation, staff spoke inappropriately to resident is deemed unsubstantiated. Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted and a copy of the report provided.
