An unannounced Complaint Investigation was conducted on this day by Licensing Program Analyst (LPA) Claudia Gutierrez regarding the allegation mentioned above. LPA Gutierrez was accompanied by Orange County Patients’ Rights Advocate, Program Manager (PM) Valerie Williams. LPA and PM met with Administrator (AD) Diosdado Mangune. LPA was taken on a guided tour of the indoors and outdoors of the facility by Office Manager Maria Nonato.

Complaint alleges staff require residents to store used toilet paper in an unsanitary manner.

Interviews were conducted with clients and AD.

Per disclosures made during interviews AD confirmed that clients are being instructed to discard used toilet paper into a trash can in the bathroom that is then discarded into a “big trash can” on the property; the clients are to take turns discarding bathroom trash can into “big trash can.” LPA observed a white 5-gallon industrial

bucket without a lid in at least two client bathrooms containing used toilet paper. LPA obtained a “schedule of bathroom trash disposal” containing a different client name for each day of the week.

Based on LPA’s observation, interviews conducted, and records reviewed it has been determined that staff require residents to store used toilet paper in an unsanitary manner. The preponderance of evidence standard has been met; therefore, the above allegation is found to be substantiated by the California Code of Regulations, Title 22, section 80087(a).

AD stated they will notify clients immediately that they are to flush all used toilet paper and a sign will be posted in every client bathroom as a reminder to flush all used toilet paper. AD will provide LPA with a copy of a statement, signed by every client, stating they have been notified to flush all used toilet
