Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced case management visit for the purpose of following-up on a report received by the Department on July 29, 2022 regarding personal rights. LPA Gutierrez met with Executive Director (ED) Anna Pastores and discussed the purpose of the visit.

On today's, visit LPA Gutierrez interviewed ED Anna Pastores, Health and Services Director Alyson Caluza and residents’ private caregiver.

There were no deficiencies issued during this Case Management visit. Based on information obtained on this date and interviews conducted, follow-up will be necessary regarding incident. An exit interview was conducted, and a copy of this report was left at the facility.
