Licensing Program Analysts (LPAs) Joseph Alejandre and Jerome Haley made an announced visit to conduct the second pre-licensing inspection. LPAs were greeted and granted entry by Licensee/Administrator Magdy Tawfik. LPAs explained the reason for the visit. LPAs and Licensee/Administrator toured the facility. LPAs observed that the following items have been corrected.

1. All 4 bedroom chairs have been replaced and are in good condition. 2. Hole in hallway closet ceiling has been repaired. 3. A new Screen Door (sliding door) in hallway next to kitchen has been installed. 4. Ceiling lighting fixture has been repaired so it hangs 8 inches from the ceiling and no longer poses a risk to potential clients. 5. Lighting fixture on side of garage by side door has been removed and a plate concealing the wires has been installed. 6. Air Conditioning (AC) unit now drains through a hose onto the lawn so water will not pool or collect next to the house or AC unit. 7. Exit path 2 on the side of the garage has been cleared so there are no obstructions or obstacles. . 8. The cable hanging from the beam connecting the garage to the house has been secured so it is mounted on the beam on no longer hangs freely next to the house. 9. All loose bricks and blocks have been removed from the backyard. 10. The table in the backyard now has an umbrella mounted to the center of the table so it now provides shade for the seating area.

All items have been corrected. LPAs did not observe any new deficiencies. The facility is ready for licensure.
During the visit LPA Alejandre explained the process of this application and also about post licensing visit once the facility is licensed. Applicant was informed today that the final approval will be processed by the CAU supervisor in Sacramento. Component III was waived during the visit due to applicant is a current licensee for a licensed ARF facility and in compliance status, supervisor approved.

An exit interview was conducted and a copy of this report, LIC809 was explained and provided to applicant Magdy Tawfik.
