32 | Per S2, staff are providing the clients with their medications as prescribed and reported that the clients have the right to refuse to take their medication.
Regarding the allegation that staff are not able to provide adequate care/supervision due to lack of staffing, the following was revealed: During the initial visit on May 6, 2026, LPA observed two staff members on duty for six clients in care. During the investigation LPA reviewed the staff schedule dated April 26, 2026, to April 30, 2026. Per staff schedule, on average there are two to three direct care staff for the morning shift from 7:00 a.m. to 3:00 p.m. and the evening shift from 3:00 p.m. to 11:00 p.m. Per staff schedule, on average there is one to two direct care staff for the night shift from 11:00 p.m. to 7:00 a.m. During the interviews with clients, C1-C2 reported that usually there are three direct care staff per shift and/or stated that one more staff would be helpful. During the interviews with staff, S1-S2 reported that they have enough staff to provide adequate care and supervision.
Based on the information gathered during the investigation and review of documents obtained, LPA is unable to ascertain if the allegations occurred as reported due to conflicting information. Although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove or refute the alleged violation occurred; therefore, these allegations are deemed UNSUBSTANTIATED.
For today’s visit, there were no citations issued per Title 22, Division 6 of the California Code of Regulations.
LPA conducted an exit interview with ED Nonoshita, and a copy of this report was provided to the facility. |
