25 | Licensing Program Analyst (LPA) Dwayne Mason Jr. arrived at the facility unannounced for the purpose of conducting a required annual inspection. LPA was greeted and granted entry into facility by DSP Virginia Abalos. Administrator Paul Chea joined the inspection approximately forty-five minutes after LPA's arrival.
The facility is a one-story home with four bedrooms, two bathrooms, kitchen, dining room, living room, backyard and attached 2-car garage. Facility appears clean, safe and sanitary. LPA noted one client was away at the hospital at the time of inspection. This client has been at the hospital since 1/28/2024 as a result of excessive behaviors. Administrator is keeping in contact with this client's case manager. Another resident was having a behavior and required the attention of the staff during the inspection. All residents rooms had required elements, including bed, chair, closet space and ample lighting. One bedroom is currently vacant. Facility has extra linens for clients in the hallway closet. Restrooms are stocked with soap and paper towels and have hand washing postings. Hot water measured at 107.9 degrees Fahrenheit and 109.4 degrees Fahrenheit in the bathrooms. LPA observed facility has emergency food and water supply. LPA observed the fire extinguisher was last serviced in June of 2023. LPA observed hazardous items such as knives, chemicals and cleaners to be locked up in cabinets in the kitchen and the garage. Knives are locked up separate from toxic chemicals. Medication for each resident is kept locked in a cabinet in the kitchen. The backyard has two shaded sitting/lounging areas. Exit gates are unlocked and self latching. LPA observed exit gates to be unobstructed. LPA observed bags of recyclable items, paint cans, unused equipment and some trash in the backyard. LPA issued a Technical Assistance advising facility to clear the aforementioned items out of the backyard. LPA reviewed all three client files and the staff files. LPA also reviewed medication and P&I for all three clients. Facility has board games, exercise equipment, electronics, music and more for client use.
No deficiencies noted during today's inspection. An exit interview was conducted and a copy of this report was provided to the facility. |
