On this day, Licensing Program Analyst (LPA) Michael Tea made an unannounced visit to complete and deliver findings for a complaint investigation. LPA Tea was greeted and granted entry by facility staff and explained the reason for the visit. LPA Tea was greeted and granted entry into the facility by House Manager (HM) Stephanie Aguilar. Administrator (AD) Oluwatosin "Joe" Adeyinka Ajijola arrived shortly to assist with the visit.

The department received a complaint on May 22, 2025, and LPA Tea conducted the initial 10-day visit a week later on May 29, 2025. LPA Tea spoke to facility staff and reviewed and collected pertinent documents and information.

It was alleged that facility does not have a qualified administrator. The investigation determined the following: It was reported that facility has no administrator. Per investigation, LPA spoke to HM Aguilar regarding their new administrator. She stated that the facility has a new administrator since the beginning

of May. Their new administrator (AD) is Mr. Oluwatosin Adeyinka Ajijola, who goes by the name of Joe. On the recently updated Personnel Report (LIC500) shows AD Ajijola listed as the facility administrator with a date employed of May 1, 2025. HM Aguilar explained that at the time in April there was no qualified administrator, the administrator for the facility had an expired administrator certificate. The licensee, Dr. Miatta Snetter, is a qualified administrator, that was spending time mainly at the other facility in La Habra.

It was alleged that staff did not meet training requirements. The investigation determined the following: LPA looked through the staff files and reviewed the staff training. 1 out of 9 staff files reviewed had current DSP training. Another staff out of 9 staff files reviewed showed different training in August 2024. Most of the staff had outdated training that was done in March 2024. As of now, since May 2025, staff have enrolled and completed their required DSP training.

Therefore, based on LPA Tea's observations, interviews conducted, and records reviewed the allegations that facility does not have a qualified administrator and staff did not meet training requirements are determined to be SUBSTANTIATED, meaning the complaint allegations are valid and that violations have occurred.

The following are being cited per California Code of Regulations Title 22 Division 6 Chapter 8.

An exit interview was conducted with Administrator Oluwatosin Ajijola and a copy of this report and appeal rights was provided to the facility.

reviewed previous monthly schedules going back till 2024. Their monthly employee schedule on the big calendar is periodically update as needed. When Regional Center Orange County (RCOC) came to the facility in April, they did not have an updated Personnel Report LIC500 at the facility. They do currently have an updated Personnel Report posted at the facility, that the administrator updates monthly.

It was alleged that facility records were not available for review. The investigation determined the following: LPA spoke to House Manager (HM) Stephanie Aguilar in which she explained they did provide all facility records to RCOC for review. Despite the records were readily available, the issue they had run into was that the required RCOC documentation were outdated or missing and they requested these documentation from their service coordinator prior to the RCOC visit. The facility were not proactive in obtaining the missing documents there creating discrepancies in their documentation. The Annual Review report by RCOC had noted that “RCOC documentation was outdated or missing, House Manager had made requests to RCOC SC. House Manager was able to locate, print and file most documents during the visit.”

Therefore, based on LPA Tea's observations and interviews conducted and records reviewed the allegations that facility did not have an updated employee time schedule and facility records were not available for review has been determined to be UNSUBSTANTIATED meaning that although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.

No deficiencies cited at this time and an exit interview was conducted with administrator. A copy of the report was provided to the facility.
