13 | Licensing Program Analysts (LPAs) Kimberly Lyman and Fred Arias conducted an unannounced complaint visit to deliver findings on the above allegations. LPAs were greeted and granted entry into the facility and explained the reason for the visit.
During the course of the investigation, LPA toured the facility and interviewed staff and residents as well as reviewed and obtained pertinent documentation such as facility menu. Regarding the allegation that licensee does not ensure menus are written at least one week in advance, the investigation revealed the following: Facility uses a two week revolving menu. LPA did not observe food items to match the noted menu during the investigation. Clients and staff interviewed stated having input into menu choices. Based on interviews conducted and records reviewed, the preponderance of evidence standard has been met. Therefore the above allegation is found to be SUBSTANTIATED.
