Licensing Program Analyst (LPA) Hanna Gough and Licensing Program Manager (LPM) A. Lucero arrived at the facility to conduct the required annual inspection and were greeted and granted entry by staff. LPA informed Administrator (AD) Aliza Festin the purpose of the inspection. The facility has four clients, three of which were present at the time of inspection while one was at work.

The facility is a one-story home with four client bedrooms, three bathrooms, kitchen, dining room, living room, backyard and attached 2-car garage. Facility appears clean, safe and sanitary. All client bedrooms had the required component and furnishings. Facility has extra clean linens for client use in the hall closet. Restrooms are stocked with soap, toilet paper, and paper towels. Hot water in the bathroom located in the bedroom next to the garage and the shared bathroom in the hallway measured between 105.6 and 109 degrees Fahrenheit. LPA and LPM observed the kitchen to be clean. LPA and LPM observed that the sharps are locked under the kitchen sink and inaccessible to clients in care. LPA and LPM observed a fire extinguisher in the dining room with a service date of February 6, 2025 and was fully charged. LPA and LPM observed the centrally stored medications were locked in a cabinet located in the dining room and is inaccessible to clients in care. LPA and LPM observed the facility has emergency food and water supply located in a closet in the hallway. LPA and LPM observed that the toxins and chemicals are locked in a cabinet in the garage and inaccessible to clients in care. The backyard has a shaded seating area for client use and is free of debris and obstructions.

LPA reviewed four staff files and no discrepancies were observed LIC 859 provided. LPA reviewed four client files and no discrepancies were observed, LIC 858 provided. LPA reviewed the P&I log with the AD and no discrepancies were observed. LPA observed the fire drills with the last drill conducted in February 6, 2025. LPA reviewed four of four client medications and no discrepancies were observed. Based on today's inspection, no deficiencies were noted in accordance with Title 22 Regulations. An exit interview was conducted with Administrator Aliza Festin and a copy of this report and appeal rights was provided.
