25 | Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation Continuation Visit. LPA was greeted and granted entry by Heidy De Leon, Program Director.
The facility is an Adult Day Program (ADP) in a single story building with an approved fire clearance of sixty ambulatory and thirty non-ambulatory clients. The facility currently has a census of forty-one clients in care; thirteen of the forty-one are non-ambulatory.
At 2:05 PM LPA toured the facility and observed clients socializing and waiting to be transported home. All clients and staff had returned from an outing at a park in West Covina with all six Vocational Innovations locations. LPA interviewed four clients regarding their quality of care and staff regarding care provided during the inspection. There are six areas in the ADP: an art room, fitness room, two skills rooms, a library and a computer area. All clients have lockers to secure their belongings while at ADP. Adjacent to the building are secured walkways for clients to have activities and were clear of obstructions.
LPA inspected the kitchen and all sharps and toxins were inaccessible and secured. The hot water temperature in the male and female bathrooms measured between 110.1 and 110.6 degrees Fahrenheit. LPA observed a separate staff/ wheelchair access restroom, a client changing room with a Hoyer Lift, the and a sensory room. There are call buttons in the library, staff/ wheelchair access bathroom, the client changing room and sensory room. The Medication Administration Record (MAR) was reviewed for two clients needing medication assistance. Per review medications are being given as prescribed. Medications are in a locked area in the file room which is inaccessible to clients and is behind the main reception area.
