25 | Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA met with Administrator (AD) Laurie Galal and Wellness Director (WD) Kim Mims and explained the purpose of the inspection.
During the inspection, LPA, AD, and WD conducted a tour of the inside and outside of the facility, common areas, resident rooms, dining rooms, kitchen, and observed the following:
The facility consists of a two-story building for assisted living and an adjacent two-story building for memory care. Select resident bedrooms were inspected and observed to have the required furnishings. LPA observed resident beds had linens and blankets. There are two courtyards, and each have a shaded sitting area. LPA observed residents socializing, engaging in leisure activities, such as listening to music, and resting in their respective bedrooms. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested between 109.9-119.6 degrees Fahrenheit. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguishers which are located at the end of every hallway were observed to be fully charged with service tags dated April 28, 2025. Kitchen appliances, and laundry washers and dryers were all inspected and observed to be operable.
Medication was observed to be centrally stored and locked. LPA reviewed centrally stored medication for select residents and did not observe any discrepancies. LPA reviewed eight resident files and five staff files. Files were observed to contain required documentation. LPA interviewed select residents and staff.
Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was left at the facility.
