13 | Licensing Program Analyst (LPA) Jeremey McClain met with licensee Mackenzie Walsh-Cheshire to deliver findings for a complaint investigation. LPA observed 12 children in care. Licensee’s assistant was present.
The following was alleged: 1)Licensee did not provide adequate supervision, resulting in a day care child sustaining multiple injuries.2)Licensee did not allow authorized representatives to inspect the facility without advance notice.3)Provider did not treat day care child with dignity.
During the investigation, LPA conducted interviews with licensee, her assistant, and parents. LPA reviewed documented evidence.
The preponderance of evidence standard has not been met; therefore, the allegations are determined UNSUBSTANTIATED. The allegations can neither be corroborated nor dismissed.
No Title 22 Deficiencies will be cited for these allegations. LPA reviewed this report with facility representative Mackenzie Walsh-Cheshire, and provided a Notice of Site Visit that must be posted for 30 days.
