Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Viktoriya Gomez for an unannounced Case Management Inspection regarding an Unusual Incident Report.

At the time of inspection, LPA observed 15 infants supervised by 5 staff members amongst three rooms.

It was reported that on 07/18/2024, a child suffered a breath holding spell while, and was transported by ambulance to the hospital where they received care. During today’s inspection, LPA interviewed staff that witnessed the incident and reviewed the child’s file. The child has since returned to care, and the staff have a plan in place to prevent the stimuli which caused the child’s reaction. It was not determined that the facility was neglectful nor was there a lack of supervision that caused the incident.

During today’s investigation, LPA discovered that unqualified staff are still providing care to children. The facility was previously cited for this violation of this regulation on 07/03/2024. A deficiency is cited on the 809-D page of this report and is a potential threat to the health and safety of children in care, if not corrected.

An exit interview was conducted, and the report was reviewed with facility representative Viktoriya Gomez. A Notice of Site Visit was provided and shall remain posted for 30 days. LPA provided Appeal Rights.

Facility is currently recruiting staff in order to have more qualified staff available. Facility representative will confirm with recruiter the qualifications that are needed for potential staff. Administration will fill in the classrooms when qualified staff are on breaks.
