32 | Interviews and file review corroborated that the windows in the 2’s classroom have needed repair for at least three years, which was known by staff at the facility, but not made known to leadership. Facility representative submitted an Unusual Incident Report on 1/16/2026, reporting that a window closed on a child’s finger, which required immediate medical attention. The full extent of the injury has not been determined. Interviews corroborated that staff attempted to open the broken window, before it slipped and closed on a child's finger.
LPA Jeremey McClain informed facility representative Esther Gamache that this report dated 1/29/2026 documents a Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Jeremey McClain informed facility representative Esther Gamache to provide a copy of this licensing report dated 1/29/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
An exit interview was conducted, and this report was reviewed with licensing representative Esther Gamache. Appeal rights were provided. A Notice of Site Visit was provided and shall remain posted for 30 days.
