On September 26th, 2022, Licensing Program Analysts (LPAs) Jeremey McClain and Lea Habtom met with Licensing Representative Andreana Thornsberry for an unannounced Case Management Inspection regarding an Unusual Incident Reported received via email on 09/23/2022.

LPAs observed 31 children supervised by five staff in three separate rooms.

There was an Unusual Incident reported regarding and alleged disciplinary method used by a staff that was not appropriate. LPA conducted interviews with staff during today’s inspection. Further investigation must be conducted to determine if a violation of personal rights occurred.

This report was reviewed with the license. LPA provided a Notice of Site Visit, which must remain posted for 30 days.
