On July 11, 2023, Licensing Program Analyst (LPA) Jeremey McClain met with director Andreana Thornsberry to deliver findings for a complaint investigation. LPA observed 11 children supervised by one school age staff. It was alleged that a staff member handled a child in a rough manner. During the investigation, LPA conducted interviews with staff and children, and made observations at the facility. Evidence was conclusive that on 05/19/2023, a former staff member physically removed a child that was laying on their stomach on top of a plastic seat, that was approximately two feet off the ground. The teacher did so after the child did not follow repeated instruction to sit on the seat properly. The child did not suffer any injuries due to this incident. The facility self-reported the incident to the Sacramento Regional Office. LPA determined the preponderance of evidence standard has been met, therefore, the allegation is determined to be substantiated.

Title 22 deficiencies are cited on the subsequent page of this report. Appeal Rights were provided, and an exit interview was conducted. A Notice of Site Visit was posted and must remain posted for 30 days.

The staff member no longer works at the facility. Director will conduct a training with all staff on responding to challenges and difficult situations with children in care. Proof of training will be sent to LPA by the POC due date: 07/21/2023.

report from 05/19/2023. On 05/19/2023, a former staff physically removed a children from a plastice seat after the child did not listen to the teachers repeated instructions to sit on the seat properly. This was considered a potential threat to the health and safety of the child at the time.
