On Tuesday, August 27, 2024, Licensing Program Analyst (LPA) Lea Habtom met with Site supervisor, Jennifer Walker, for the purpose of a required annual inspection. Facility is located on the Franklin Elementary School Campus in portable #23. All criminal record clearances are associated to STAR Oakhills #313600444. Facility hours of operation are Monday through Friday from 2:30pm - 6:00pm. There were 44 school age children outside being supervised by 4 staff and the site supervisor.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA toured the classroom and outdoor play area. Medications were reviewed. Toxic and hazardous items are inaccessible to children. Furniture and equipment are in good condition. Facility shares the school restroom and playground. All waivers are being followed. The floors appeared clean throughout the facility. All food was protected against contamination. Drinking water was readily available to children both indoors and outdoors by a water cooler and individual labeled water bottles. There are no firearms or bodies of water on the premises. There is a functional carbon monoxide detector. All staff and six children's records were reviewed. At least one staff member present today has current Pediatric CPR and First Aid certification. LPA reviewed the Department's inspection authority and discussed any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s personnel, and administrative records. There were no Title 22 Deficiencies cited during today's inspection.

Exit interview conducted and report was reviewed with the site supervisor Jennifer Walker. A notice of site visit was given and must remain posted for 30 days. Appeal rights provided.


Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
