25 | On May 11, 2027, Licensing Program Analyst (LPA) Stephanie Piring met with Facility Representative,Angie Schiavone, for the purpose of conducting a Case Management Inspection. During todays visit, LPA observed 58 napping children supervised by 4 staff across 4 classrooms.
Sacramento RO received an Unusual Incident Report from the facility self-reporting an absence of supervision incident that occurred on May 4, 2026. During today's inspection, LPA made observations, and interviewed the Assistant Principal regarding the incident. LPA determined by incident report and interview that a child was found in the bathroom by a staff member. The facility reports that the child may have snuck past staff while taking a child to the restroom, when exiting the restroom, staff closed the bathroom door, the child was found less than a minute later by a different staff member taking a different child to the bathroom. Facility has retrained staff on supervision, transitions, name to face including classroom and bathroom sweeps when transitioning. Facility will implement daily "transition observation checklist" conducted by Administrative team.
Based on Today’s inspection, One Title 22 deficiency is being cited on 809-D and an immediate civil penalty of $500 is being assessed.
LPA Stephanie Piring informed facility representative, Angie Schiavone, that this report dated 5/11/26 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Stephanie Piring informed the facility representative to provide a copy of this licensing report dated 5/11/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview was conducted and a copy of this report was given to Facility Representative Angie Schiavone. Notice of site was provided and must remain posted for 30 days. Appeal rights provided.
