32 | Evidence gathered corroborates that on 03/06/2026, Staff 1 exhibited inappropriate behavior by throwing toy items at Child 1, in response to their behavior. Evidence gathered also corroborates that Staff 1 used inappropriate language in the presence of children in care while they were involved in a verbal altercation with a parent.
Furthermore, evidence corroborates that Child 1 was left without the presence of a staff member, as they were able to climb over a chain link fence near the facility without staff in the vicinity. .
Although LPA was unable to corroborate the cause of the injuries Child 1 suffered, the preponderance of evidence standard has been met, and the allegations are SUBSTANTIATED.
LPA Jeremey McClain informed facility representative Kristie Brantley that this report dated 04/06/2026 documents two Type A citations which shall be posted for 30 consecutive days as there were immediate risks to the health, safety, or personal rights of children in care.
Also, LPA Jeremey McClain informed facility representative Kristie Brantley to provide a copy of this licensing report dated 04/06/2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
An exit interview was conducted, and this report was reviewed with licensing representative Kristie Brantley. Appeal rights were provided. A Notice of Site Visit was provided and shall remain posted for 30 days. |
