On July 28, 2025 Licensing Program Analysts (LPA) Stephanie Piring met with Facility Representative Banafsheh (Bonnie) Lee to deliver complaint investigation findings. Upon arrival, LPA observed 18 infant children, supervised by 5 staff members across 2 classrooms.

It was alleged that Staff do not ensure Infant changing table is in good repair. During the the investigation, LPA conducted Interviews with the director/owner and staff, and made observations. LPA observed a changing pad that had been worn from repeated use and cleaning during LPAs visit on 6/3/25. This concern was brought to the attention of the director and the changing pad was replaced on the spot. Based on observation the preponderance of evidence standard has been met; therefore, the above allegation is SUBSTANTIATED.

An exit interveiw was conducted and this report was reviewed with Facility Representative, Banafsheh (Bonnie) Lee. A notice of cite was given and must remain posted for 30 days. Appeal rights were provided.

On July 28, 2025 Licensing Program Analysts (LPA) Stephanie Piring met with Facility Representative Banafsheh (Bonnie) Lee to deliver complaint investigation findings. Upon arrival, LPA observed 18 infant children, supervised by 5 staff members across 2 classrooms.

It was alleged that staff do not provide adequate supervision to children in care. During the course of the investigation, LPA made observations, and conducted interviews with the director, and staff. During LPAs visits on 6/3/25, and 7/28/25, LPA observed adequate supervision of children in both the infant and toddler classroom.

It was alleged Staff do not ensure cleaning agents are inacessible to children. Interveiws with staff stated cleaning agents are stored in baskets above the sink areas out of reach of children. LPA observed the storage of cleaning agents in the classrooms and with in the facility and confirmed cleaning agents are stored where they are inacessible to the children.

It was alleged, Staff do not ensure toys are properly sanitized. Staff state that toys that have been used by children are placed in a bucket by the sink, to be cleaned and sanitized. LPA observed the bucket with dirty toys for later sanitizing and confirmed that they were placed out of reach of children and no longer available for play.

It was alleged that staff are operating out of ratio. During LPAs visits on 6/3/25, and 7/28/25, LPA observed both the infant classroom and toddler classroom operating with in ratio.

Although the allegations may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is UNSUBSTANTIATED. Exit interview was conducted and report was reviewed with Facility Representative Banafsheh (Bonnie) Lee . Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.

LPA brought the worn changing pad to the directors attention and the changing pad was changed immediatly during LPAs visit on 6/3/25.
