Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Teresa Cortes to deliver findings for a complaint investigation. LPA observed 35 children supervised by five staff.

It was alleged that staff in care do not have proper training, and that staff yelled at children in care.

During the investigation, LPA conducted interviews with staff and parents, reviewed staff files, and made observations of classroom interaction between staff and children.
Evidence did not corroborate the allegations, as there was evidence to support that all staff complete training on various aspects of child care before working at the facility and continue training throughout their tenure. Evidence did no suggest that staff yell or have yelled at children in care.

The preponderance of evidence standard has not been met; therefore, the allegation is determined to be UNSUBSTANTIATED. The allegation can neither be corroborated nor dismissed. Continued...

Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Teresa Cortes to deliver findings for a complaint investigation. LPA observed 35 children supervised by five staff.

It was alleged that staff are not meeting the diapering needs of children in care, staff inappropriately handled a child in care, and a staff member fed children after feeding themselves without washing their hands or put on gloves.

During the investigation, LPA conducted interviews with staff and parents, reviewed staff files, and made observations of classroom interaction between staff and children and observed footage of teacher’s classroom interactions with children in the past.
Based on evidence gathered, the allegations are corroborated.

Interviews were conclusive that multiple parents have picked their child up with a full diaper and there were multiple occurrences of children going home with diaper rash. (1/2)

LPA observed footage of a former staff placing a child in their seat in a rough manner and pushing them toward the table. Evidence was also conclusive that this same staff was also observed moving children in a manner that was inappropriate by grabbing them by their jacks or sweaters. Another former staff was also observed pulling a child up by one arm in a manner harmful to the child.

The facility admitted that on once occasion, a staff member fed themselves a food item with their fingers and then proceeded to serve children food without washing their hands or putting on gloves.

The preponderance of evidence standard has been met; therefore, the allegations are determined to be SUBSTANTIATED.

Deficiencies are documented on the 809-D pages of this report. An exit interview was conducted with facility representative Teresa Cortes. A Notice of Site Visit was provided and shall remain posted for 30 days.

LPA Jeremey McClain informed licensee Tere Cortes that this report dated 05/12/2025 documents one Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Jeremey McClain informed licensee Tere Cortes to provide a copy of this licensing report dated 05/12/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

LPA and director discussed the possibilty of having another teacher in the Early Preschool Classroom designated for diaper changes. A plan will be developed to ensure that diaper changes occur in the afternoon - early evening before pickup. Facility will notify parents of diaper rashes and ask for

with a full diaper. Evidence was also conclusive that multiple were observed to have diaper rash after being picked up from the facility. This poses a potential risk to the health and safety of children in care if not corrected.
