25 | Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Pamela Nasca for an unannounced Case Management Inspection regarding an Unusual Incident Report.
No school age children were present during the inspection.
It was reported that on 10/15/2024, a child(C1) suffered a fractured foot after they dropped from the monkey bars on the playground. C1’s parents were contacted and C1 was diagnosed later in the evening when they went to the emergency room. During today’s inspection LPA conducted an interview with staff, observed the play area where the incident happened, and reviewed the original incident report. LPA observed the cushioning/bark to be sufficient under the monkey bars and did not observe the grip on the bars to be slippery or hazardous.
An exit interview was conducted, and the report was reviewed with facility representative Pamela Nasca. A Notice of Site Visit was provided and shall remain posted for 30 days. |
