13 | LPA Parks arrived on Thursday January 25, 2023 to conclude a complaint investigation regarding the above allegations. Prior to the visit, LPA completed the required COVID-19 testing protocols and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 mask.
LPA discussed the allegations with Licensee Larisa. Throughout the course of the investigation, LPA reviewed documents pertaining to R1 and interviewed staff. LPA obtained a copy of 30-day notice to vacate from R1’s POA. Per Administrator and Licensee, no eviction notice was ever given nor was the resident refused readmittance once discharged from hospital. Through interviews with the Licensee and Administrator, both denied that resident was unable to return to the facility. All staff stated that all medication is kept locked in a cabinet in the kitchen, inaccessible to residents in care. Additionally, when LPA toured resident rooms, no medications were observed outside of locked cabinet in kitchen.
